How to create a Digital Form

Learn how to build and customize a new digital form. This guide covers the basics, such as adding various question types, setting field requirements, and configuring titles and descriptions for your custom forms

1. Log in to Rezoomo

2. Click on the Setup Guide in the bottom left corner

3. Click "Digital Templates"

4. Click "Add Template"

5. Click "Digital Form"

6. This is where you build your digital form. Start by giving it a name—click “Add Form Title (Internal)”.

This title is for internal use only and won’t be visible to candidates.

7. Click “Add Title and Description” to add a title and description that will be visible to candidates.

8. To move the Title field (or any question), click on the field, then drag it from the highlighted area to your desired position in the form.

9. Create your first question by typing into the highlighted “Untitled Question” field.

10. Type your question, then use the dropdown to select the Question Type.

11. In this example, I’ll use the Short Answer question type, since I only need the candidate’s IBAN and BIC-a long response isn't required.

Click Save.

As you add more questions, make sure to click Save for each one.

12. Click on the question above where you want your new question to appear, then click Add Question.

13. Add your question as shown earlier. In this example select Contact Number as your question type. Then click Save

14. To limit the length of a candidate’s response, you can set a word limit. Candidates will see how many words they have remaining as they type.

15. You can make a question mandatory or optional by toggling the Required option to Yes or No.

If a question is set to required, candidates must answer it before they can submit the form or application.

16. To duplicate a question, click the Duplicate Question button.

17. To delete a question, select it and click the Delete icon.

18. To add a description to a question, click the Actions menu (three dots), then enable Description to add your text.

19. Enter your text in the Description field and click Save

20. Your form is now ready to use. You can preview it at any time using the Preview button, or test it using the Test button.
